Outpatient Therapy - Financial Request Process Home – Ministries – Care Ministries – Outpatient Therapy - Financial Request Process
What it is
Members and regular attenders may receive church financially-assisted counseling for a variety of situations, depending on fund availability.
How do I access it?
Payment for counseling may be provided following an initial session with one of the pastors. The pastor will walk you through the process:
- If you are in crisis, which is defined as a life-threatening situation of imminent danger, any pastor may authorize a single session counseling evaluation.
- For on-going counseling, an Outpatient Financial Assistance Request Form is completed and processed by the Pastor of Community Life. This form contains two sections, a financial information section, and requestor agreement section, whereby the requestor agrees to be held accountable to attend the sessions and make an honest effort with the counseling if it is approved. Because individual circumstances vary, the number of sessions, duration of the therapy and co-pay amount appear as blanks on the form and are completed with guidance from the pastor, up to a maximum of 10 counseling sessions. Since participation in counseling is enhanced when a co-payment is made, a minimum co-pay amount of $25 per session is desired.
- If sessions are missed or if the requestor otherwise does not fulfill the agreement to participate in therapy, the counselor is required to notify the Pastor of Community Life. The church will not be responsible for the expense of missed sessions.
- After completion of the approved number of sessions within the agreed time frame, if further counseling is needed, the scope of the church's involvement is re-evaluated. This can include more treatment, more financial budgeting, follow-up meetings with a pastor and/or ministry involvement.
- At any point in the process alternate support can be implemented if determined to be appropriate, such as the use of a Stephen Minister, prayer, small group participation or assistance from a Deacon.
